Important Information About Your MedDataCard™
In order to make your MedDataCard™ more
effective, it is recommended that you add additional files to the card.
These files should be in a form that most computers will be able to
read. The use of MS Word® or other word processing files is not
recommended, since you cannot assume that all computers will have these
specific programs. The use of a text file will insure the readability by
all PCs and MACs. Text files can be created in the operating systems
program "Notepad". Scanning documents into Adobe Acrobat® format (.pdf),
or graphic formats (.jpg, or .gif), will also insure readability.
Additional items to include are:
- Recent EKG
- Last blood test results
- Pacemaker or defibrillator data sheet
- ID information for implanted replacement
parts (i.e. Hip Joint,
Knee, etc.)
- Signed Organ Donor Authorization Form
